The Client Questionnaire is a tool we use to collect the initial information from you that we'll need to get started with your case. We usually have all of our clients complete it because it's a good way to gather the basic info for most cases. If there's something that doesn't apply to your situation, feel free to skip it. It shouldn't take more than 5-10 minutes to complete.
